Pictures and attachments do not use the $SETTINGS[:url], and therefore assume that the wiki is running at the site root (which is usual) and not being redirected from a subdirectory.
ie:
Running the wiki at http://soks.counsell.org works, but running it at http://soks.counsell.org/wiki wouldn’t.
